The SpaceX OSHA crane collapse has triggered federal scrutiny after a hydraulic crane failed at the Starbase facility in Texas. The agency also found that another crane at the site had operational computer issues before the incident. These findings suggest lapses in equipment readiness during a high-risk cleanup operation.

Separately, the agency continues to investigate a December accident involving a subcontractor employee who alleged he was crushed by a falling metal support.
